A layer,
not a volume.

Most people asking about liposuction ask the same question: how much can you take out? It is the wrong question. Fat is not a quantity to be reduced — it is a layer with depth, and the shape of a body is decided by how that depth varies across it.

VASER is the instrument that makes that layer workable. Ultrasound energy emulsifies and separates fat before anything is aspirated, and because the fat is already released, the layer close to the skin can be approached under control rather than torn through to get there. That is the whole of its contribution, and it is a large one. The technology is what makes the design possible; it is not the design itself.

This page is about VASER — ultrasound used as energy, to separate fat — together with the design method, the fees by area and the recovery. Three related subjects have pages of their own, because they are genuinely different questions:

VASER separates the fat. It does not remove it. The removal is done by a second instrument, a powered vibrating cannula, and that instrument answers a different problem — keeping a broad surface even. VIBROFIT (PAL) has its own page.

Ultrasound guidance is not the same as ultrasound energy. VASER, described here, uses ultrasound to emulsify fat. UGRAFT refers to working under real-time ultrasound imaging, so that the layer being treated is seen rather than judged by feel. And high-definition athletic etching designed around male anatomy is a discipline of its own, with different targets and a different conversation about maintenance.

Principle

Shadow and highlight, not volume.

Conventional liposuction removes fat “uniformly” across a treatment area to avoid irregularity. It is a safe instinct, and it produces a predictable outcome: a smaller version of the same flat surface. It also erases the undulations that make a body read as fit.

The alternative is to treat the fat layer the way a sculptor treats stone — as a medium with depth. Thinning it along the border of the rectus abdominis or the external oblique deepens a groove that already exists, and the groove reads as shadow. Leaving fat over the muscle belly preserves a highlight. Neither works without the other. Remove everything and you have contrast with nothing to contrast against.

The discipline of this work is restraint — knowing how deep to carve, and where the etching has to stop for the result to remain believably yours.

Instruments

Ultrasound, used as energy.

Liposuction performed by hand alone depends heavily on the surgeon’s movement. Over a small, detailed area that is an advantage. Over a broad one it is the origin of unevenness, because no hand keeps a perfectly constant rhythm for an hour. So we do not rely on a single instrument — and this page describes the first of the two.

What VASER does, and what it does not

Ultrasound energy emulsifies fat and separates it selectively before aspiration. Because the fat is already released, the cannula can work close to the dermis under control rather than tearing tissue to get there. This is what makes shadow and highlight possible — and what suits detailed regions: abdominal definition, the chest, the jawline.

The instrument itself is a solid titanium probe, not a hollow cannula — it removes nothing. Energy leaves it at the tip and, on grooved probes, along rings cut into the shaft. The number of those grooves is a deliberate choice made before the case begins. More grooves spread energy sideways and reduce what is delivered straight ahead, so fragmentation becomes gentler and broader — which is what a soft, fatty layer needs. Fewer grooves concentrate energy at the tip, which is what dense fibrous tissue requires before it will release at all: the male back, gynaecomastia, an area that has been operated on before.

Output is set as an amplitude, and the energy can be delivered continuously or in pulses. Pulsed delivery is what makes superficial work defensible. Close to the dermis the margin for thermal injury is narrow, and interrupting the energy allows the tissue to cool between pulses. Continuous delivery belongs in the deep layer and in fibrous tissue, where the difficulty is releasing the fat at all rather than protecting the skin above it.

The endpoint is a loss of resistance, not a stopwatch

Emulsification is complete when the probe stops meeting drag and begins to pass through the layer freely. That is a tactile endpoint, judged continuously, and it arrives at a different moment in every patient and every area. It is not a preset number of seconds.

Continuing past it does not remove more fat — nothing is being removed yet. It only deposits energy into a layer that has stopped needing it, and that is precisely where the recognised thermal risk of energy-based liposuction comes from. Knowing when to stop is more of this technique than knowing how to start.

Before any energy: the wetting solution

Nothing is switched on in a dry field. The layer is first infiltrated with a dilute solution containing local anaesthetic and adrenaline, and the infiltration has to be complete rather than approximate before the probe is introduced. The fluid expands the layer, constricts small vessels, and acts as the medium the ultrasound disperses into. Ultrasound applied to an under-infiltrated field is the classic route to a burn, because the energy has nowhere to go.

The volume is planned against the volume expected to be removed from that area, not applied by habit. Adrenaline also needs time to take effect, so the interval between infiltrating an area and beginning work on it is part of the operative plan rather than a delay in it.

Two small things at the skin edge, which matter more than they sound

Every access port is fitted with a skin protector, and the skin around it is kept wet throughout. The probe passes back and forth through that one small opening hundreds of times in a case.

The port — not the depth of the layer — is where the skin is most likely to be injured, and a mark at a port is one of the few complications of this operation that is visible permanently. The precaution costs nothing.

VASER is never the whole operation

An emulsified layer is still inside you. Something has to remove it, and that something is a different instrument with a different problem to solve. Where VASER decides what is possible close to the skin, the aspirating cannula decides whether the surface it leaves behind is even.

Design

Drawn standing, before anything is touched.

The outcome is largely determined before the first incision. In conventional liposuction the preoperative step is marking — a pen line tracing the area to be treated. Here it is closer to the drawing a sculptor makes before the first chisel strike, which is why we call it design.

Your skeletal landmarks, your muscle attachments, and the thickness and layered structure of your subcutaneous fat are mapped and translated into a plan drawn on your body: where shadows deepen, where highlights are preserved, where volume may need to be added rather than removed. Reasoning backward from the finished silhouette, each decision is recorded in several colours of ink.

One principle is non-negotiable

The design is drawn while you are standing. Surgery happens supine or prone — but you are seen, and see yourself, upright.

When the body reclines, gravity releases its hold on the breasts, hips and abdomen, and fat shifts from its natural position. A design drawn on a reclining body risks a mismatch between plan and result. Drawing under the actual load of gravity is also what reveals muscle tone, side-to-side asymmetry and lifelong postural habit — the things that decide whether the work translates into a natural line when you stand up again.

Strategy

The design differs by frame, and by sex.

The idea that thinner is automatically better has no place in this work. Skeletal width, muscle attachment patterns and fat distribution vary enormously between patients, and the goal is to draw out the healthy appeal already present in your own anatomy.

Female design

A smooth hourglass, and an elegant abs crack

The priority is the continuous, flowing curve from shoulder to waist to hip and into the thigh — a softly rounded hourglass. At the abdomen we do not manufacture a sharply segmented six-pack. We evoke a refined vertical midline instead, so that gentle contrasts of light and shadow, rather than hard edges, define the curve.

Contrast before continuity

The emphasis inverts: clearer structural lines, deeper shadow, more pronounced dimension. The targets, the depth of work and the maintenance conversation all differ enough that male contouring is treated as its own discipline rather than a variation of this one.

The abs crack, and why it is made by leaving fat behind

In female abdominal design we do not engineer a sharply cut six-pack. The aim is a soft, refined vertical line down the centre of the midsection — often called an abs crack — that suggests the muscle beneath rather than displaying it.

Anatomically it is made by working the deep subcutaneous layer along the linea alba and along the lateral borders of the rectus abdominis, casting subtle shadows that describe the central line. What matters just as much is what is not done: the superficial layer is not removed wholesale. It is deliberately left in place, where it functions as a highlight and preserves the soft surface texture that reads as feminine.

This is the opposite instinct to the one that produces male definition

A hard, over-etched abdomen on a female frame is not a stronger version of the same result. It is a different result, and usually not the one that was wanted. The refinement lies in how much superficial fat is left, not in how much is taken.

Candidacy

Who it suits, and who it does not.

Likely to suit you if

  • You are close to a stable weight and want to change a shape, not a number
  • You have localised fat that has not responded to diet or training
  • Your skin has good elastic recoil, so it can re-drape onto the new contour
  • If you want visible definition, there is muscle under the fat for the design to reveal
  • You understand that the result responds to how you live afterwards
  • You are in good general health, with low anaesthetic risk

Not appropriate if

  • You are looking for weight loss — liposuction is not a treatment for obesity
  • Your weight is still changing significantly, or you plan a major change soon
  • Your skin has lost its recoil; excess skin needs excision, not suction, and suction alone would make it look worse
  • You want definition that your musculature does not support — the result would read as engineered rather than natural
  • You have significant heart, lung or clotting disease

Where a different plan would serve you better — skin excision, fat grafting to add volume rather than remove it, or simply waiting until your weight is settled — that is what you will be told at consultation, before you commit to travel.

The Operation

What actually happens.

  1. Design, standingSkeletal landmarks, muscle borders and fat thickness mapped and drawn on your body, in several colours, under gravity. Where shadow is to be deepened and where highlight is to be preserved are two different sets of lines, and they are drawn as such.
  2. Anaesthesia and positioningGeneral anaesthesia for body contouring of this scope; the plan is confirmed after pre-operative blood tests. Circumferential work requires you to be turned during the operation, which is one reason the design has to remain legible in more than one position.
  3. Access portsA small number of punctures, sited where the mark will be hidden and where the geometry lets an area be reached from more than one direction. Crossed access matters: a layer worked from a single angle carries the signature of that angle in the surface.
  4. Infiltration, and waitingThe treatment layer is infiltrated with a dilute solution of local anaesthetic and adrenaline until the field is fully expanded. Then time is allowed for the adrenaline to act. Beginning early costs blood; beginning in an incompletely infiltrated field risks a burn.
  5. Ultrasound emulsificationThe probe and its groove pattern are chosen for the tissue in front of them — fewer grooves for fibrous areas, more for soft ones — and the energy is pulsed rather than continuous where the work is close to the dermis. The endpoint is the loss of drag against the probe, not a set time. Ultrasound here is energy, not imaging.
  6. Aspiration with VIBROFIT (PAL), deep layer firstThe powered cannula aspirates the released fat at a constant rhythm, taking the deep layer with a larger bore before anything superficial is touched. Over broad areas this constancy is what keeps the surface even.
  7. The etching — selective work in the superficial layerThe smallest cannula, openings turned away from the skin, following the drawn shadow lines along the borders of the muscles. Fat is deliberately preserved over each muscle belly. This is the part that is design rather than removal, and it is where restraint decides the result.
  8. Pinch assessment, not visual assessmentLayer thickness is checked between finger and thumb and compared with the same point on the other side. A supine, swollen, infiltrated body does not look symmetrical even when it is — the pinch is the more reliable instrument at this moment, and the drawn design is the reference.
  9. Harvest, where fat is being moved rather than discardedWhere grafting to the breasts, hips or pectorals is part of the plan, the graft is harvested in this same operation and handled differently from the moment it leaves the body. Ultrasound-guided placement is quoted separately.
  10. Final symmetry check against the designSide compared with side, and both compared with what was drawn before anything was touched.
  11. Ports and compressionThe punctures are managed to drain rather than closed tight — retained infiltration fluid is a cause of prolonged swelling. A compression garment is fitted before you leave theatre. The skin has to re-drape onto a new contour, and compression is how that is guided.

Recovery

Two weeks in Japan, six months to the final line.

The compression garment is part of the operation

Removing volume leaves the overlying skin with more surface area than it needs. Compression is how that skin is guided back onto the new contour — it is not a comfort measure, and it is not optional.

Worn as instructed, it reduces swelling, limits seroma and helps the surface settle evenly. Abandoned early, it is one of the more common reasons a technically good result reads as irregular at three months.

Evidence

What the published data shows.

You should be able to see the numbers rather than be told that a technology is better. These are from peer-reviewed studies, and we have included the ones that failed to find a difference alongside the ones that did.

Randomised, single-blind, contralateral trial — 20 patients, 33 paired anatomical regions, VASER against conventional suction liposuction[1] Skin retraction 17 per cent per litre on the VASER side against 11 per cent per litre on the conventional side — a 53 per cent relative improvement (p = 0.003). Blood in the aspirate 11.2 against 14.0 cc per 100 cc, a 26 per cent reduction (p = 0.019). At six months, neither patients nor surgeons could tell the two sides apart on pain, swelling or appearance.
Comparative series, back and posterior flanks — 27 conventional against 30 VASER[2] Haemoglobin in the conventional aspirate 7.5 times higher, haematocrit 6.5 times higher (p < 0.0001). The authors conclude the difference matters most in tight, fibrous areas and in large-volume cases.
Retrospective series, single surgeon, 15 years — 261 patients, 783 areas, minimum six-month follow-up[3] Overall complication rate 4.6 per cent: contour irregularity 1.9 per cent, seroma 0.8 per cent, cellulitis 0.8 per cent, pigmentation change 0.4 per cent, electrolyte imbalance 0.4 per cent. 86 per cent would recommend the procedure.
Prospective series of abdominal etching — 25 patients[4] No haematoma, infection or seroma. One superficial pressure wound, caused by a dressing applied badly rather than by the surgery. 96 per cent happy or very happy at six months. A small series, and a favourable one.
Prospective comparison — 35 patients, VASER against a second ultrasonic system and against conventional liposuction[5] No significant difference in the blood content of the aspirate between any of the three methods. This directly contradicts the two studies above, and we would rather you saw it than did not.
Randomised intraindividual comparison — 13 abdomens, VASER on one half against a different ultrasound system on the other, followed to 24 months[6] No significant difference in measured skin elasticity, distensibility or hydration between the two devices. Skin firmness favoured the comparator. Patients could not tell the halves apart.

These figures describe published study populations. They are not results at this practice and they are not a prediction of your individual outcome. Full citations are at the foot of this page.

Two things this table does not show, and one it does

It does not settle the question. The randomised trial and the back-and-flank series found real advantages in skin retraction and blood loss; the two most recent comparisons found none. That is the actual state of the literature, and a page claiming otherwise would be choosing its citations.

It contains nothing about VIBROFIT. Power-assisted liposuction has not been subjected to comparative trials in the way ultrasound has. The case for it is mechanical and practical — a constant rhythm over a broad area, less force through the tissue — and we present it as exactly that rather than dressing it as evidence.

What the table does show is the complication profile, consistently, across thousands of areas: the risk of this operation is contour irregularity, not catastrophe. Which is the same reason restraint, and not power, is the part of the technique that matters.

Frequently asked.

Is liposuction a way to lose weight?

No, and treating it as one is the most common reason patients are disappointed. Liposuction changes shape, not weight. The right candidate is close to a stable weight already and wants to change a contour that diet and training will not change. If your goal is a number on the scale, this is the wrong operation.

How is VASER different from ordinary liposuction?

VASER uses ultrasound energy to emulsify fat before it is aspirated. Practically, that means controlled work is possible in the superficial layer close to the skin — which is what allows etching along the borders of muscle rather than uniform removal from the deep layer alone. The technology is what makes the design possible; it is not the design itself.

Is VASER the same as ultrasound-guided liposuction?

No, and the two are easily confused because both involve ultrasound. VASER uses ultrasound as energy, to emulsify fat before it is aspirated. Ultrasound-guided contouring uses ultrasound as imaging — a live picture of the layer being worked, so that depth and plane are seen rather than judged by feel. They are complementary rather than alternatives, and they answer different questions.

Do you use VASER on its own?

Usually not. VASER and VIBROFIT (PAL) do different jobs and most of our cases use both. VASER separates the fat so that detailed, near-dermal work is controllable; VIBROFIT aspirates it at a constant mechanical rhythm, which is what keeps a broad area even. Put simply, one shapes a plane and the other builds dimension. Which leads depends on the area being treated and on what the design asks for.

Why is the design drawn standing?

Because that is how you are seen. Surgery happens lying down, but when the body reclines, gravity releases its hold and fat shifts from its natural position. How long do I need to stay in Japan?

Plan for at least two weeks after surgery. That period covers early recovery, compression management and at least one in-person follow-up before you are cleared to fly. Your arrival date before surgery is set separately in your written plan.

Fees are charged by area, from ¥275,000 per area including tax; the complete abdomen is ¥770,000 and HD abdominal etching is ¥1,210,000. What is an “abs crack”, and can I have one?

It is the soft vertical line down the centre of the abdomen — a refined midline that suggests the muscle underneath rather than dividing it into segments. It is created by thinning the deep layer along the linea alba and the lateral borders of the rectus abdominis while deliberately preserving the superficial layer as a highlight. Whether it suits you depends on your skin quality, how much visceral fat sits behind the abdominal wall, and the tone of the rectus itself — all of which are assessed at consultation rather than assumed.

Does the fat come back?

The fat cells removed do not regenerate. However, the cells that remain can still enlarge if you gain weight, and they will do so across your whole body, including the treated areas. The shape you are given is stable; the condition it sits on is yours to maintain.

How is the amount of ultrasound decided?

By the tissue, not by the clock. Emulsification is complete when the probe stops meeting resistance and begins to pass through the layer freely, and that moment arrives differently in every patient and in every area. Applying energy past it removes no additional fat, because nothing is being removed at that stage. It only deposits heat into a layer that has stopped needing it, and the recognised thermal risk of energy-based liposuction comes from exactly that.

Is the evidence for VASER as strong as the marketing suggests?

No, and it is worth being clear about it. A randomised trial found 53 per cent better skin retraction and 26 per cent less blood loss than conventional liposuction, and a series of 261 patients reported an overall complication rate of 4.6 per cent. But two more recent comparisons found no significant difference in blood loss or in skin properties, and in that same randomised trial neither patients nor surgeons could tell the two sides apart at six months. The technology makes a particular kind of design possible. It does not perform the design, and it is not the reason a good result looks the way it does.

What does the one-year guarantee cover?

VASER liposuction and VASER 4D carry a guarantee for one year after surgery. It includes re-examination by your operating surgeon at no consultation charge, post-operative treatment such as management of pain, swelling and wound care, and — from six months after surgery, at the surgeon’s discretion — design adjustment. Medication and anaesthesia may be charged separately, and using the guarantee means returning to Japan.
