RIBXCAR reshapes the lower rib cage instead of resecting it. Through punctures rather than an incision, and under real-time ultrasound, the outer cortex of the lower ribs is scored with an ultrasonic instrument and the ribs are angulated inward. The bone stays. Its shape changes.
It exists because some waists cannot be narrowed with liposuction. If the distance and angle between your lower ribs and your pelvis leave no room for a curve, removing fat will not create one — and patients who have already had liposuction and still see a straight waist are usually describing a skeletal limit, not a surgical failure.
RIBXCAR is a skeletal waist procedure that narrows the waist by scoring the outer cortex of the lower ribs with an ultrasonic instrument and angulating them inward under real-time ultrasound, rather than resecting them.
Origin
RIBXCAR was developed by Dr. Raúl Manzaneda Cipriani in Lima, Peru, founder of AX — Aesthetic Xpert Scientific Lab. Dr. Higuchi did not invent it. He performs it as AX Asia Ambassador, and teaches it to other surgeons in Japan through the seminar programme AX runs.
We say this plainly because it is worth more to you than a claim of invention would be. Skeletal waist contouring is performed by a small number of surgeons worldwide, techniques are not standardised between countries, and the single most useful question you can ask any surgeon offering it is where they were trained in it, and by whom. Here the answer is: by the organisation founded by the person who developed it.
Candidacy
Careful selection is the strongest safety measure this operation has. The criteria below come from the published series, and are assessed individually rather than applied as a checklist.
Being told you are not a candidate is a legitimate outcome of a consultation, and sometimes the right one. Where a different plan — soft-tissue contouring alone, or weight optimisation first — would serve you better, that is what you will be told.

Once the ribs have been remodelled and the deep fat layer cleared, the soft tissue has to readapt to a new framework. External compression holds the ribs in their narrower configuration while that happens.
In a prospective series of 328 patients, of the 12.5 per cent who reported pain, more than half of that pain was attributable to misuse of the corset rather than to the fracture itself. If you are not going to wear it as instructed, this is not the right operation for you — and it is better to establish that before surgery than after.
Evidence
You should be able to see the numbers rather than be told the operation is safe. These come from peer-reviewed studies, and we have included the least flattering one as well as the favourable ones.

Read the table again and notice what the serious complications are. They are not aesthetic complications. They are thoracic ones.
Before he performed a single aesthetic case, Dr. Higuchi spent eighteen years in thoracic surgery and lung transplantation. The chest wall and the pleural space were his daily working anatomy for most of his surgical career, and recognising and managing a pneumothorax was routine rather than exceptional. That is not a marketing point. It is the specific competence this particular operation happens to require.

One point specific to this procedure: it is not covered by the one-year guarantee that applies to VASER liposuction, VASER 4D, Condense Rich breast augmentation and gynecomastia surgery. We would rather tell you that here than let you assume otherwise.
Risks
This is elective surgery on the skeleton. It is substantially more invasive than soft-tissue contouring, it is not covered by health insurance in Japan, and its outcome cannot be guaranteed.

No. RIBXCAR reshapes the lower ribs rather than removing them. A corticotomy is made on the outer cortex only, the rib is angulated, and the inner cortex and periosteum are left intact, so the thoracic cage keeps its structural continuity. Older approaches to waist narrowing removed the eleventh and twelfth ribs; this is a different operation.
The technique is performed through percutaneous punctures rather than an incision, so it does not leave the scar associated with open rib resection. Where liposuction is combined with it, the usual small liposuction access points also apply.
In a multicentre study of 3,805 patients, the mean waist reduction was 11 cm and the mean angular reduction across ribs 10, 11 and 12 was 10 degrees. That describes an average across a study population, not a promise. Your own result depends on your skeleton, your soft tissue and your skin, and is estimated individually at consultation.
Plan for at least two weeks after surgery. That period covers early recovery, management of the external fixation, and at least one in-person follow-up before you are cleared to fly. Your arrival date before surgery is set separately in your written plan.
The surgical fee is ¥2,200,000 including tax. General anaesthesia, the standard post-operative set and blood tests are additional. Rib remodeling is usually combined with deep-layer liposuction of the waist, which is quoted separately. Your written quotation states the total.
It is a real operation with real risks, and the serious ones are thoracic — pneumothorax and haemothorax. In the multicentre series of 3,805 cases there were four pneumothoraces and one haemothorax; an independent survey of several rib remodeling techniques put pneumothorax at 0.17 per cent of cases. These are uncommon but not zero, and patient selection and surgeon experience are what keep them uncommon.
Published criteria exclude a body mass index of 30 or above and visceral fat of 13 per cent or more, and the operation is not appropriate where skin elasticity is poor, where there is significant heart, lung or clotting disease, or where a previous thoracic condition makes work near the pleura unwise. Willingness to wear the post-operative compression as instructed is also a genuine criterion, not a formality.
